A Case of Atypical Bronchial Carcinoid with Ectopic ACTH Syndrome

The differential diagnosis and management of Cushing's syndrome remain difficult, particularly for ectopic adrenocorticotropin (ACTH) syndromes. Bronchial carcinoid tumor is an extremely uncommon but well recognized cause of ectopic Cushing's syndrome. We have experienced a case of ACTH-producing atypical bronchial carcinoid tumor in a 68-year-old female presenting symptoms of generalized weakness and moon face. Clinical presentation, radiologic and hormonal evaluation, and pathologic examination were compatible with ACTH-producing atypical bronchial carcinoid tumor. The immunohistochemical staining of the tumor showed positivity for ACTH. Surgical resection of the tumor resulted in clinical remission and correction of hypercortisolism and normal plasma level of ACTH levels.
