A Case of Pre- and Postoperative Analgesia by Continuous 3-in-1 Nerve Block in Patient with Femoral Neck Fracture

Abstract

Patients who have suffered from femoral neck fracture require urgent pain relief and this is most effectively achieved by 3-in-1 nerve block. The technique is simple, requiring no special positioning, and because of the favorable anatomical location of the femoral nerve it has a particularly low complication rate. The prompt application in the acute situation considerably reduces the symptoms and consequences of traumatic shock. The 3-in-1 nerve block enables painfree positioning of the patient for performing spinal and epidural anesthesia. In this case a catheter was placed in the ipsilateral femoral nerve sheath, and 3-in-1 nerve block was induced with injection of 30 ml 0.25% bupivacaine through the catheter, followed by continuous infusion of 0.125% bupivacaine at 10 ml/h. The 3-in-1 femoral nerve block is preferable to continuous epidural blockade for pre- and postoperative analgesia.
