Henoch-Schlein Purpura Associated with Acute Pancreatitis and Gastric Hemorrhage

Abstract

Henoch-Schnlein purpura mainly occurs in children, and may accompany gastrointestinal, renal, joint or cutaneous symptoms. Gastrointestinal symptoms include nausea, vomiting, andominal pain, gastrointestinal hemorrhage, intussusception, intestinal perforation, and pancreatitis. However, acute pancreatitis is rarely associated with Henoch-Schnlein purpura. Recently, we have experienced a case of Henoch-Schnlein purpura complicated with gastric hemorrhage and acute pancreatitis in a 20-year-old female patient. She complained of coliky abdominal pain, lower extremity purpura, and melena. The patient received corticosteroid therapy of 1 mg/kg dosage to treat persistent abdominal pain, melena, and progression of hyperemic edematous mucosa in endoscopic finding. The corticosteroid therapy improved patint's symptoms dramatically. The patient was discharged at 15th hospital day, and has been followed up without recurrence.
