The study of conscious sedation in both mandibular 3rd molars extraction in Koreans

Abstract
- Conscious sedation places patients in a pain-free state and reduces their discomfort during operations. It has been used successfully during minor surgeries and endoscopies. However, the proper dosages drugs to be used on korean patients during minor oral and maxillofacial surgeries such as surgical extractions are not
yet known. As a result the administration of an improper dosage on korean patients has been linked, possibly, to unexpected deep sedation and postoperative complications including nausea, vomiting and dizziness. The purpose of this study was to determine proper sedative and dosages to be used in conscious sedation of korean patients during oral and maxillofacial surgery. The patients were assigned to one of the following group: group1(n=20), midazolam+fentanyl; group2(n=21), propofol+fentanyl. Following the application of sedatives, analysis on sedation level, pain, and postoperative complications were done on Korean patients who were subjected to the extraction of both mandibular 3rd molars, between the periods of September 2001 and February 2002, at St. Mary's hospital, the Catholic university of Korea. The BIS level was dropped significantly at 5minutes after adminstration, but raised at odontomy procedure. BP & pulse change was similar to BIS. Pain was checked on 50% in midazolam group, 38% in propofol group. Amnesia was shown 40% in midazolam group, 38% in propofol group. Complications(headache, dizziness, nausea) were checked 20%, 21%, 7% in midazolam, 21%, 21%, 3% in propofol. The results state that experimental dosage of both sedatives did not reveal a significant difference. So we need ongoing experiment to find effective dosage. We present our clinical experience with literature review.
