Clinical Observation on Benign Prostatic Hypertrophy

A clinical observation was made on forty-eight cases of benign prostatic hypertrophy, admitted to the Department of Urology, Seoul Red Cross Hospital during the period from January 1, 1974 to December 31, 1978. The results were summarized as follows. 1) The incidence of benign prostatic hypertrophy was 8.1% to 596 total in-patients and 35.0% to male in-patients 50 years old or more. 2) Eighth decade was prevalent (58.3%) with mean age of 72.1 years old. 3) Mean period passed away from initial symptoms to visit was 2.4 years. Common symptoms and signs were dysuria with small caliber of urinary stream in 38 cases (79.2%), urinary retention in 27 cases (56.3%) and frequency in 17 cases (35.4%). 4) Mean volume of residual urine was 420ml. 5) I.V.P. was performed in 39 cases, and hydronephrosis was found in 5 cases(12.8%). 6) Endoscopy was performed in 35 cases. The common findings were trabeculations(90.0%) and inflammations(84.8%). 7) Pulmonary diseases (20.8%) and hypertension (16.7%) were frequently associated with B.P.H. 8) Methods of operative treatment was retropubic prostatectomy in 25 cases 9) Mean weight of removed adenoma was 32.2 gms. 10) Mean period of gross hematuria was 4.1 days and of indwelling catheter was 9.4 days. 11) Histopathologically, B.P.H. with inflammatory signs were frequently seen (64.0%), and occult carcinoma was found in 1 case (4.0%). 12) Mean amount of blood transfusion was 2.08 pints. 13) Postoperative complications were urinary infections in 16 cases (64.0%), temporary incotinences in 5 cases (20.0%). intravesical hematomas. urethral strictures. wound abscesses and pyrexia (each 8.0%). The mortality rate was 0%.
