Clinical Observation of Klebsiella Pneumonia in Adults

Abstract

Clinical observations were made on 28 cases of Klebsiella pneumonia, who were admitted to the Dept. of Chest Medicine, National Medical Center from Jan. 1979 to Dec. 1985. Following results were obtained. 1) Male to female ratio was 2: 1 and in 2 nd decade of life the highest incidence was noted. 2) The most common subjective symptoms were fever with chill, cough, and chest pain. 3) The predisposing factors and associated diseases were found in 50% of patients. Pultbc. was the most common associated disease and alcoholism τvas noted in 18% of patients. 4) As for the amount of daily sputum, 54 % of patients showed scanty sputum. The characteristic of sputum was tenacious and difficult to raise in 75% of patients. The color of sputum was whitish in 46% and blood-tinged & chocolate in 38% 5) In roentgenologic findings, bronchopneumonia or lobular pneumonia was about twice of lobar type. Right-to-left ratio was 2 : 1 and the most frequently involved area of the lung was right middle lobe. 6) The sensitivity test for Klebsiella bacilli showed all sensive to Amikin , Gentamycin, Cefoperzone and Cefotaxime. 7) All cases except one were improved completely or partially by selective antibiotics, repeated bronchoscopic aspirations and postural drainage. 8) 7 cases (25%) developed the complication: pleural effusion (18%) lung abscess (7%) ,pneumothorax (4 %) and empyema(4% ) in order of frequency. 1 case expired during hospitalization and the mortality rate was 3.5%.
