Clinical Experience with Hoke's Traction

Abstract

In the management of fractures of the shaft of the femur in children, it is still a standard practice at most centers in the world to employ Bryant's traction until three, five or even six years of age and then to employ Russells traction beyond that age. It has been pointed out recently, however, that the upper age limit within which Bryant's traction may be safely employed is two years instead of three, five or six years of age as previously suggested, and that the lower age limit within which Russel's traction may be effectively employed is six years of age. In our experience, Hokes traction is the safest and ideal method between two years and six years of age. Children between six years and twelve years of age may be treated optionalIy either by Russells traction or by Hoke's traction, although we prefer the latter. Children beyond twelve years of age may be treated either by Russells traction or by skeletal traction as in adults. Five cases of fractures of the shaft of the femur in children treated with Hokes traction at S.N.U.H, during the past two years, from 1964 through 1966, are presented. Hoke's traction has not been a very popular method in the treatment of fractures of the shaft of the femur in children, but we believe it is a simple, useful and convenient method in seleted cases.
