Renal oncocytoma of distal convoluted tubular origin: A case report including immunohistochemical study and review of literatures

Renal oncocytoma is an uncommon, generally benign tumor arising from tubular epithelial cells. This tumor shows typical gross, microscopic and ultrastructural features, but the origin of this tumor. whether proximal or distal tubule. has remained controversial. In Korea five cases have been documented. but little pathologic details with immunophenotypic characteristics of this neoplasm are available to clarify the origin. A round tumor was detected in the left kidney from a 57 year-old man by ultrasonography and computed tomography. The resected kidney contained a 4.5 cm-sized, single, mahogany-brown colored. solid and round tumor bulging out from the renal capsule. Microscopically the tumor was made up of large polyhedral cells with acidophilic and granular cytoplasm. Ultrastructure showed packing of slightly uneven mitochondria. The tumor cells showed strong immunoreactivities to epithelial membrane antigen(EMA) and cytokeratin. but not to vimentin, alpha-1-antitrypsin, alpha-1-antichymotrypsin. or lysozyme. The adjacent nonneoplastic proximal tubules were negative and distal tubules were positive to EMA. The aboves support that the renal oncocytoma arises from the distal convoluted tubular epithelial cells.
