Multiple primary lung cancers, either synchronous or metachronous, are unusual. We report 65-year-old man with double lung squamous carcinoma in the distal trachea and right lower lobe segment. He with long history of smoking was admitted to the hospital because of a weight loss and chest discomfort in the right lower lobe. A chest X-ray film on admission showed a soft mass in the right lower lung field. CT scan revealed T3N3M0 (stage IIIB) lesion. Examination with a fiberoptic bronchoscopy revealed a distal tracheal exophytic tumor and a exophytic tumor almost completely obstructing the right lower segmental bronchus. Biopsies were done, and histological diagnoses of both tumors were respectively squamous cell carcinoma. He has being treated by chemotherapy.
