A Case of Hysteroscopy Assisted Therapeutic Abortion of Pregnancy in a Side of Uterus Didelphys Affected with Obstructed Hemivagina

We report a very rare case of a pregnancy which occurred in one side of uterus didelphys affected with ipsilateral obstructed hemivagina. The patient was referred to Samsung Medical Center after failure of 2 consecutive trials of elective abortion at 7th gestational week at private clinic under the impression of ectopic pregnancy. The presence of uterine anomaly was suspected during ultrasound guided sounding of uterus, and was confirmed as uterus didelphys and obstructive hemivagina with cervicovaginal fistula by MRI and HSG. Therapeutic abortion was performed by partial incision into vaginal septum and hysteroscopic evaluation followed by dilatation and curettage. The incised vaginal septum was left open. This patient became pregnant again in the same uterine side and delivered a female baby of 2.73 kg birthweight at term pregnancy by cesarean section due to breech presentation.
