Aritifical Insemination of Oligo-astheno-teratozoospermia

Abstract
- The lower conception rate or pregnancy rates among OATS (Oligoasthenoteratozoospermia) effected couples are mainly due to the therapentic difficultly and ineffectiveness of conventional therapeutic techniques. This study is based on the semenalysis of the partners of the infertility clinic patients. Following results were obtained from the manuplation of the sperm concentration, motility, and artificial insemination in order to increase conception rate. 1. Of 639 cases of semenalysis, 32.2% were normal. Among the abnormal findings, 18.5% of oligospermia, 20.6% of oligoasthenozoospermia, 8.5% of OATS, 19.2% of azospermia and 0.9% with all dead sperms were found. In comparison, semenalysis of healthy donor group showed 74.5% of normospermia and 25.5% with abnormal findings. 2. Etiologic study of abnormal semenalysis shows the idiopathic origin being the most common cause in all cases. Varicocele, congenital, vas ligation and vas reversal failure are the next leading causes of abnormal semenalysis in decreasing frequency. 3. Semen washing technique to improve motility and concentration was effective in the cases of over 20 millon sperm counts with the improvement of motility to above grade III. However in cases of below 5-10 milon sperm counts, there were no significant improvement in motility, after the procedure.
4. Improvement of motility after cryopreservation is depended on the initial sperm concentrations. In cases of over 40 millon sperm count, grade III or above were scored as 55% in fresh sample, 50% in thawed sample, and 70% in washed sample. In contrast, 50%, 40% and 65% were scored in cases of below 20 millon. And only 35%, 20% and 30% in cases of below 10 millon. 5. Successful pregnancy following artificial insemination (1 to 3 cycles) with normal semen is 35% compare to 15.6% in oligospermia in women aged between 20 to 29 yrs. In women between age of 30 to 39 yrs. 37.5% with normal semen and 20.1% with oligospermia. Trial of artificial insemination following semen cryopreservation shows 30.4% success rate with normal sperm, and 14.3% with oligospermia.
