Contralateral Epidural Hematoma Developing after the Craniotomy for Traumatic Intracranial Hematoma.

Abstract

During a 10 year period, we experienced sixteen patients of acute contralateral epidural hematoma that developed after craniotomy for traumatic intracranial hematoma. 14 cases had linear skull fractures ipsilateral to the acute epidural hematoma, which might have injured the dural artery. The mechanism of this complication was not clearly understood, but it was suspected that bleeding of the injured dural artery ensued from the abrut release of the mass effect caused by the intracranial hematoma. When sudden brain swelling during the craniotomy without obvious reason or a change for the worse on neurological finding after that is encountered, affention should be directed to acute epidural hamatoma developed from the fracture site on the contralateral side.
