A Case of Henoch-Schnlein Purpura Complicated with Lower Gastrointestinal Bleeding Treated with Dapsone

Abstract

A patient with Henoch-Sch nlein purpura showed hematochezia and cutaneous lesions on endoscopy. Upper gastrointestinal endoscopy revealed mucosal redness and petechiae at the duodenal bulb and antrum. The colonoscopic findings showed edema, petechiae, erythematous and purpuric lesions in the descending colon and rectosigmoid area. Systemic steroid was used but failed to control the intestinal symptoms and skin lesions. Therefore, dapsone therapy was initiated at a dosage of 100 mg daily with regular check of the blood methemoglobin level. After 15 days, the patient's symptoms were improved and the patient has been well for the past 3 months. This result suggests dapsone may be an effective drug for uncontrolled gastrointestinal bleeding in Henoch-Schnlein purpura.
