Vasoactive Drug Self-Injection into Corpus Cavernosum in Erectile Impotence

Abstract

Elderly men (mean age: 55 years from 24 years to 78 years) with organic impotence were treated with intracavernous injection of vasoactive drug combinations. Possible causes of these patients were diabetes mellitus (18 patients), vasculogenic (10 patients), antihypertensive drug (11 patients), climacteric (14 patients), low hormonal level (8 patients), advanced age (9 patients), post-surgery (5 patients), post-trauma (3 patients), Peyronie's disease (2 patients), congenital curvature of penis (1 patient), Klinefelter's syndrome (3 patients) and psychogenic (4 patients). One ml (30mg) of papaverine hydrochloride mixed with phentolamine mesylate (1mg) was injected into one side of corpus cavernosum of 88 patients with organic erectile impotence. Coital penetration was possible in the 78% patients (88.6%) following physician-administered trial injection of the vasoactive drugs. After the injection, erection started in 6 minutes, maximal rigidity of penis attained in 20 minutes, and erection status sustained for 138 minutes. Fifty-six patients who were successful at intercourse after trial injection, requested to have the vasoactive drugs for self-injection at home. They were received 10 ml of the vasoactive drug combination with instruction sheet following they agreed to sign informed consent. At periodical follow-up investigations, they were proved to be satisfied with successful intercourse using self-injection methods without any untoward side effects. Of these, 24 patients had 10 self-injections for 1-3 months, 16 patients had 20 self-injections for 2-5 months, 7 patients had 30 self-injections for 6 months, 3 patients had 40 self-injections for 7-8 months, 2 patients had 50 self-injections for 7-9 months, 1 patient had 70 self-injections for 10 months, 1 patient had 90 self-injections for 12 months, 1 patient had 110 self-injections for 15 months, and 1 patient had 200 self-injections for 24 months. There were minor complications such as ecchymosis on the injected site of prepuce but subsided spontaneously in a few days. Priapism occurred as major complication in 3 patients after trial injection. Unclotted blood was aspirated from 2 patients. Erection was subsided spontaneously in one patient 19 hours after the injection. Intracavernous injection with vasoactive drug combination may be useful transitory treatment for the erectile impotence.s
