Scrotal Flap with Z-plasy Technique in Extensive Penile Paraffinoma

Abstract

We report our experience with a scrotal flap with Z-plasty for the correction of extensive penile paraffinoma. Thirteen patients suffered from distressing complications of paraffin injection, including penile skin necrosis and palpable masses. The paraffinoma was confined within the penile skin in 11 patients and extended to the whole penile skin with focal involvement of the upper scrotum in 2 patients. A single-stage surgical procedure was performed to resolve the problem. We proceeded with complete resection of the denuded penile shaft with bilateral scrotal flaps. Additionally, two large Z-plasties were incorporated at the ventral and dorsal suture lines of the flaps. At follow-up 2 to 15 months postoperatively, all patients remained free from the serious problems such as penile contracture, flap necrosis, and wound infection that may be seen with other surgical techniques. Furthermore, the cosmetic apperance of penis was satisfactory in all patients. We believe that the scrotal flap with Z-plasty is worthy of application to the penis that has been extensively denuded by a foreign body or trauma.
