Abstract

We reviewed retrospectively 52 germ cell testis tumors managed at SNUH during the last l5 years. There were 33 children (16 yolk sac tumors, l3 teratomas and 4 mixed) and l9 adults (9 seminomas and 10 non-seminomatous germ cell tumors). Six patients had cryptorchism. Serum AFP and HCG were evaluated in 45 patients. In 96% (26/27) of the non-seminomatous germ cell tumors except teratoma either AFP or HCG was elevated. Eight seminoma patients treated with radiation therapy were free of disease for over Z years and one who had refuse radiation therapy had recurrence. In Eighteen children (6 yolk sac tumors, 9 teratomas and 3 mixed) were followed-up for over 2 years and l6 cases are disease free. resulting an over-all survival rate of 89%. Ten adult non-seminomatous germ cell tumors (5 stage A, l stage B and 4 stage C) were treated with orchiectomy alone (4), orchiectomy and PVB regimen (4) and orchiectomy and radiation (2). 9 patients were followed-up and only two patients are alive. This poor result of treatment can be attributed to : 1) half of them are in the advanced stage with mixture of choriocarcinomatous elements, 2) understaging and inadequate treatment in the initial cases of this study, 3) poor compliance of patients, 4) two referred cases from other hospitals after orchiectomy. were too late for adequate treatment.
