Association between Normal Weight Central Obesity and Cardiovascular Disease Risk Factors in Middle-Aged Men

Abstract

Background
Obesity and Central obesity increases the risk of cardiovascular disease (CVD). Recent studies have shown that normal weight central obesity (NWCO) is associated with CVD risk factors and suggested that the waist-to-height ratio (WHtR) is effective predictive index for CVD. The purpose of this study was to examine the association between NWCO, classified using body mass index (BMI) and WHtR, and CVD risk factors in middle-aged Korean men.
Methods
The subjects for this cross-sectional study included 11,917 males aged 40-64 years who visited the hospital for a health check-up. Anthropometric measurements and biochemical studies were conducted. The subjects were categorized into the following four groups using BMI and WHtR: normal weight (BMI 18.5-24.9 kg/m 2 ) and no central obesity (WHtR <0.5) (NW), normal weight and central obesity (WHtR ≥0.5) (NWCO), obesity (BMI ≥25 kg/m 2 ) and no central obesity (OB), and obesity and central obesity (OBCO). A logistic regression model was used to calculate the odds ratio and 95% confidence intervals (CI) for CVD risk factors such as hypertension, dyslipidemia, and diabetes.
Results
With reference to NW, the adjusted odds ratios for hypertension 1.461 (95% CI, 1.268-1.683, P<0.001), dyslipidemia 1.802 (95% CI, 1.543-2.103, P<0.001), and diabetes 1.475 (95% CI, 1.205-1.804, P<0.001) were significantly higher in NWCO.
Conclusion
NWCO was associated with CVD risk factors, such as hypertension, dyslipidemia, and diabetes, compared with NW. It is important to focus on NWCO for the prevention of CVD in Korean middle-aged men.
