Impact of Mitral Regurgitation on the Hyperemic Induction by Intravenous Administration of Adenosine

Abstract

We present a case of an intermediate coronary artery stenosis with concomitant severe mitral regurgitation, in which hyperemic efficacy of intravenous (IV) infusion of adenosine was initially blunted and was completely restored by surgical correction of the diseased valve. As the half-life of adenosine is extremely short, any conditions resulting in a low cardiac output state may hinder the accumulation of serum adenosine given intravenously enough to induce maximal hyperemia. In this situation, alternative methods to increase IV infusion rate or switch to intracoronary injection may be advisable.
