Uncommon Terminal Ileum Diverticulitis for Right Lower Quadrant Pain

Abstract

Terminal ileum diverticulitis is a very rare disease that may lead to an acute abdomen mimicking appendicitis. Most patients are asymptomatic and diagnosis is made on routine imaging studies. The acute complications of ileal diverticula including inflammation are very rare. The management of ileal diverticulitis is similar to that of colonic diverticulitis. It can treat conservatively if recognized on imaging prior to laparotomy. Some patients need surgical intervention because of complication. A 37-year-old man visited emergency room because of right lower quadrant pain. Abdomen ultrasound showed small sac-like outpouching lesion with increased echogenicity of surrounding fat in thickened terminal ileum, suggesting inflamed diverticulum and sonogenic tenderness. The patient was treated and recovered after terminal ileum diverticulitis was diagnosed.
