Introducing Patient Safety Activities in Canada: Focused on Medication Incidents and Preventions

Abstract

Patient safety (PS) in healthcare organizations is an important problem worldwide. A patient safety reporting and learning system has been proposed as a means to keep patients safe. Canada operates the ‘Canadian Medication Incident Reporting and Prevention System (CMIRPS)’, which collects and manages medication accidents at the national level and carries out preventive activities based on them. Organization participating in the CMIRPS are Health Canada (HC), Canadian Institute for Health Information (CIHI), Canadian Patient Safety Institute (CPSI), and Institute for Safe Medication Practice (ISMP) Canada. The HC supports the budget for the operation of this system and provides knowledge from the perspective of federal regulators of health products. The CIHI is in charge of the National System for Incident Reporting (NSIR), which collects patient safety incidents from medical institutions in Canada. The CPSI is the World Health Organization (WHO) collaborating centre for patients safety and patient engangement, and its representative activity is the Global Patient Safety Alerts (GPSA), which collects and provides information on domestic and international patient safety incidents. ISMP Canada has a separate reporting and learning system for individual healthcare professionals, community pharmacies and patient and care givers. Therefore, we would like to overview the current status of risk management in Canada related to medication and identify the implications that can be introduced to Korea.
