Tuberculous Emyema with Muitiple Bullous Change in Diabetic Male

Abstract

A case of tuberculous empyema which was thought as bullous destruction of lung due to bacterial pneumonia, was observed. This 46-year-old male, newly found diabetic was admitted with the complaints of fever and productive cough of 20 days duration- Serial chest film showed rapid change from parenchymal infiltration of right upper lobe to multiple bullous change of entire right lung with air-fluid levels. Extensive bacteriologic examination including transtracheal aspiration failed to reveal any causative organism. After conservative management with antibiotics and physical therapy, right pneumonectomy was performed for totally destroyed right lung. Pathologic specimen revealed extensive parenchymal tuberculosis and tuberculous empyema.
