Clinical Study of Transient Synovitis of the Hip Joint in Children

Abstract

A fairly common cause of painful hips in children, which subside in short time by bed rest is the condition variously known as transitory arthritis of hip joint, transient coxitis serosa seu simplex, acute transient epiphysitis and transient synovitis. It is now felt that the term transient synovitis most aptly describes the condition, because the pathology seems to lie entirely within the synovial membrane of the hip joint. The pathology is of interest, not because it is a disabling condition, but because of the difficulties encountered in differentiating it from more serious disease of the hip. Of the 73 cases so diagnosed at the Dong San Presbyterian Hospital of Taegu from 1963 to 1969 20 were reviewed: all the remaining cases were eliminated from this study because of inadequate followup. Following observations were made: 1. The patients ages ranged from 2 to 13 years, the average being 6.7 years. 2. 15 of the 20 cases were boys and 5 were girls. 3. There were no significant findings on laboratory examinations in all the cases of the series. 4. Swelling of the gluteus minimus shadow was the most consistent finding on roentgenographic examinations of the pelvis. 5. Differential diagnosis from diseases of the spine and the knee as well as from other more serious diseases of the hip was a necessary prequisite in reaching acurate diagnosis. 6. Non-weight bearing regimen was continned for 2 weeks after symptoms had subsided.
