Sonographic feature of solitary small echogenic lesions in the liver: diagnostic algorithm focusing onhemangioma and hepatoma

Abstract

Twenty-five solitary small (less than 5cm) echogenic lesions in the liver were retrospectively analysed which were collected from July 1983 to Oct. 1986 at yonsei university young done severance hospital. The results asfollows; 1. Lesions consist of 11 hemangioma, 11 hepatocellular carcinoma, 1 pseudotumor in fatty liver, 1 cystadenoma and 1 metastatic lesion from stomach carcinoma. 2. Hemangiomas show strong echogenic foci in all caseswhich is contact with liver capsule in 6 cases out of 11 cases. Echotexture of hemangioma was hemogenous in 9cases. There show no peripheral halo in 10 cases. 3. Hepatocellular carcinoma was associated with cirrhosis in allcases. Peripheral sonoluscent halo was noted in 10 cases. Lesions over 3 cm show heterogenous textures in 5 out of6 cases. Lesions below 3cm could be hyperechoic, hyperechoic or isoechoic. 4. Hypoechoic lesion anterior to portalvein which was seen in fatty liver, disappeared after improvement of fatty liver which suggest pseudotumor. One case of cystadenoma show unusual strong echogenic focus. One case of echogenic focus revealed metastatic malignancy from stomach carcinoma.
