Percutaneous transhepatic variceal obliteration of intractable bleeding of gastroesophageal varices

Abstract

Percutaneous transhepatic variceal obliteration (PTVO) is a safe, effective, and palliative means of treatment in bleeding gastroesophageal varices, in cases of conservative treatment failure, massive bleeding, gastricvarices which cannot be controlled with endoscopic sclerotherapy, & high surgical risk group as Child C. We have performed 67 case of percutaneous transhepatic variceal obliteration in 60 patients with intractable variceal bleeding from July. 1985 to January 1989 at Kyungpook National University Hospital. The results obtained were asfollows. The age of patients was ranged from 11-65 years, old, most frequent in 5th & 6th decade(77%). Male to female ratio was 3.5:1. Successful bleeding control was obtained in 60 cases(90%). Esophageal varices were 33 cases(50%), gastroesophageal varices, 20 cases (30%), and gastric varices, 13 cases(20%). Procedure one yearsurvival rate was 55%. Postprocedure one year rebleeding rate was 55%. PT should be considered as an effective treatment of bleeding gastroesophageal varices. Particularly in cases of gastric varices in which endoscopicsclerotherapy is difficult and high surgical risk group, it must be the treatment of choice.
