Abstract
- We have treated 51 patients for penile sclerosing lipogranuloma caused by a foreign substance such as paraffin or Vaseline. According to the extent of the lipogranuloma, we classified the patients preoperatively into three groups : lesion confined to the distal penile shaft(type l), extension to the mid or whole penile shaft(type ll), and extension to the glans penis(type lll). All 18 patients with type l lipogranuloma and 8 to the 10 patients with type ll lipogranuloma underwent successful primary closure of the skin defect after complete excision of the lesion. Scrotal advancement flap penoplasty was performed to cover the skin defect in 17 of 18 patients with type ll lipogranuloma and 2 of 4 patients with type lll lipogranuloma in whom secondary repairs were required because of would disruption. With staged bipedicled scrotal flaps, we treated 3 patients with type lll or type ll lipogranuloma without failure. A bipedicled scrotal flap was the recommended procedure for patients with large skin defects and lipogranuloma extending to the glans penis.
