A Case of Tinea Corporis Caused by Trichophyton tonsurans in an American Girl Living in Korea

A 5-years-old caucasian girl had had a well demarcated scaly erythematous papular lesion, 13 x 10cm in size on the left buttock for 6 months. She had lived in Korea for 2 years. A KOH examination from the lesion showed many hyphae. Cultures from the scales of the lesion were done on Sabouraud dextrose agar tubes at 25 degrees C. Many slowly growing, mahogany colored colonies with suede like surfaces were cultivated. The growth rate of the colony was inhibited on trichophyton agar I (caseine agar), compared to that on trichophyton agar III (caseino agar+ thiamine and inositol). On microscopic examination, there were abundant, large, tear drop or club-shaped microconidia of various sizes, on open branching clusters or thickened terminal hyphae. Characteristic gross findings of the colony and typical microscopic findings confirmed Trichophyton tonsurans. The lesion was cleared with topical terbinafine cream after 1 month of treatment.
