The Application of Endoscopic Mucosectomy in Various Mucosal Lesions of Gastrointestinal Tract

Abstract

BACKGROUND
In case of submucosal tumor, adenoma and atypism, we can't frequently use the snare polypectomy for treatment of them. Adenoma and atypism are premalignant, so other therapeutic modalities are necessary for resection of them.
METHODS
We have performed endoscopic mucosectomy such as strip biopsy, and "O" ring ligation on various mucosal lesion of GI tract in 60 patients(32 men, 28 women) from January 1996 to October 1997. Sixty patients who had adenoma or polyps were followed up endoscopically for 4 months to 24 months.
RESULTS
The results were as follows: Sixty patients(79 lesions) were enrolled in this study. Mean age was 54.8 years(range 19-88 years). The method of mucosectomy was strip biopsy in 57 cases(75 lesions), and "O" ring ligation in 3 cases(4 lesions). The rates of complete resection were 98.0% in upper gastrointestinal tract and 93.3% in lower gastrointestinal tract. No serious complication occured such as massive bleeding or perforation.
CONCLUSION
Based on the above results, the endoscopic mucosectomy is considered to be a safe and effective modality in the treatment of the flat polyps of gastrointestinal tract and could be used for prevention of gastric cancer.
