Abstract

The preferred methods of surgical management of the groin hernias in adults have been changed as the time passes. Recently new biocompatible prosthetic materials are introduced and minimally invasive operations using laparoscopy are popular. These advances make the surgical procedures more diverse and have challenged the classical procedures using the anterior approach and the autogenous tissues in augmentation of the hernia defects. In the era of cost-effectiveness in managed care and the quality of life of the patients, postoperative discomforts and recovery as well as long-term recurrences are important points to choose the optimal operative procedures. Basically the hernia is an anatomical defect and so the topographical anatomy of the groin is reviewed. The advantages and disadvantages of the various surgical procedures now performed by many surgeons are discussed. Currently tension-free mesh-plug hernioplasty appears to be the procedure of choice for the majority of groin hernias in adults.
