Studies on Factors Associated with Pancreaticojejunostomy Leak after Pancreaticoduodenectomy

Abstract

The pancreaticoenteric anastomosis leakage after pancreaticoduodenectomy is a source of considerable morbidity and potential mortality. The incidence of anastomotic leak after pancreaticoduodenectomy ranges from 8 to 18%. The aim of this study was to determine the perioperative factors that influence clinical outcomes in patients with a leaking pancreaticojejunostomy after pancreaticoduodenectomy. From January 1985 to December 2001, 187 patients underwent pancreaticoduodenectomy at Chonnam University Hospital of Korea, for a variety of indications. We encountered 19 pancreaticojejunostomy leaks (10.2%). They were 12 men and 7 women with median age of 60.1 years (male: 57.1 years, female: 65.2 years). Age and sex of the patients, serum bilirubin level, serum albumin level, use of PTBD, intraoperative PRC transfusion, chronic pancreatitis, and the stage of tumor did not seem to contribute for the development of leakage from the pancreaticojejunostomy. Pancreaticojejunostomy leak was fewer in the group of patients who used the stent in pancreatic duct as compared with those not used the stent in pancreatic duct although this difference was not significant (p=0.08). The study suggests that perioperative parameters does not affect the pancreaticojejunostomy leakage. Futher study on operative parameters such as emergency operation and methods of pancreaticojejunostomy anastomosis may be needed.
