Aim: We designed this study to evaluate the meaning of preoperative indocyanine green(ICG) R15 test comparing with clinical parameters and surgically resected specimens in patients of hepatocellular carcinoma with underlying chronic liver diseases. Materials and Methods: Forty-one cases who underwent hepatic resection in the Department of Surgery, two Hallym University Hospitals during the period between March 1993 and November 1998, were admitted to this study. Relationships between conventional liver functions, histological activity index(HAI), ICG R15, Child's and Child-Pugh's classifications were estimated. Results: ICG R15 values were significantly correlated with Child-Pugh's classification, HAI and serum albumin level(p<0.05), but no significant correlation was seen between Child's classification and ICG R15. Two patients died of postoperative liver failure within 1 month inspite of their ICG R15 and operative extent being 10 %(trisegmentectomy), 13 %(monosegmentectomy) respectively. Conclusion: We concluded that ICG R15 value might be a very useful test to reflect clinical functional reserve of the liver and histological grading of chronic liver diseases, but additional parameters including CT volumetry and oral glucose tolerance test would be necessary to evaluate liver functional reserve appropriately.
