Ulnar Neuropathy Caused by a Schwannoma in the Guyon's Cannal

Abstract

Guyon's canal syndrome is a compression neuropathy of the ulnar nerve entrapment at the wrist. Compression of the ulnar nerve at the wrist by a ganglion, lipomas, diseases of the ulnar artery, fractures of the hamate and trauma is a common etiological factor. We report on a rare case of ulnar neuropathy caused by a schwannoma at the level of Guyon's canal.

  • Figure 1 A about 2.2×1.4 cm sized homogeneous mass in Guyon's canal over the anterior portion of left pisiform is seen.

  • Figure 2 A well capsulated yellowish colored mass, 1.7×1.0 cm in size inside the epineurium of the ulnar nerve is identified when nerve sheath was incised, and the mass was enucleated.

  • Figure 3 (A) Excised specimen was a yellowish round rubbery mass, measuring 2.2×1.5×0.8 cm. (B) The tumor is composed of spindle cells with wavy appearing nuclei (H&E, ×100). The mass is strongly positive with S100 protein. (C) There were more celluar "Antoni A" pattern with looser stroma and fewer cells "Antoni B" pattern (H&E, ×200).
