Therapeutic Effect of Amniotic Membrane Transplantation in Active Bacterial Corneal Ulcer

Abstract

To evaluate the efficacy of amniotic membrane transplantation in the management of active bacterial corneal ulcer. Nineteen patients (19 eyes) with acute bacterial ulcer that had persisted despite topical and systemic antibiotics medication were transplanted of amniotic membrane (1~3 layers). The amniotic membrane was soaked in antibiotics before transplantation in all cases. Thirteen eyes had hypopyon. The mean time before amniotic membrane transplantation was 10.89+/-6.50 days, and the mean area of the corneal ulcer was 8.94+/-6.56 mm2. Visual acuity was improved in 9 eyes (41.4%). In 6 eyes (31.6%) the visual acuity after the surgery was similar to the one before the surgery. In 12 eyes, corneal ulcer was healed in 3 weeks and 4 eyes persisted epitheial defect. 3 eyes with corneal ulcer progression 2 of 3 eyes developed phthisis bulbi and 1 of 3 eyes had evisceration of globe. Amniotic membrane transplantation seems to be a useful adjunctive surgical procedure for the management of active bacterial corneal ulcer by inhibiting the bacteria and reducing inflammation.
