A Case of Pyogenic Liver Abscess with Spontaneous Subcapsular Hemorrhage

Abstract

Pyogenic liver abscess is a serious clinical problem, which gives rise to complication in 30-50% of the cases. Spontaneous subcapsular hemorrhage is a rare complication associated with liver abscess. A 30-year-old woman was admitted with right upper quadrant abdominal pain, febrile sensation, and myalgia for 3 days. A septated 6 X 6 mg sized ill-defined round and low-density lesion with hematoma was observed in subcapsular area of both hepatic lobe. Hepatic arteriography showed stretching of the branches of the right hepatic artery without extravasation of the contrast material. Ultrasono-guided percutaneous transhepatic aspiration was performed for subcapsular hematoma. The aspirates had blood count of WBC 58,500/mm(3), hemoglobin of 7.8 g/dL, and platelet of 20,000/mm(3). The aspirates of low density lesion was pus, in which Klebsiella pneumoniae was isolated. Liver biopsy was performed around the abscess and there was no evidence of hepatic neoplasia. After treatment of antibiotics for six weeks and percutaneous transhepatic drainage, she recovered completely. We report a rare case of pyogenic liver abscess complicated with spontaneous subcapsular hemorrhage.
