Hemospermia is a not uncommon entity in urologic clinics. The exact pathophysiology has not been elucidated, but abnormalities of the prostate and seminal tract are suspected. Transrectal ultrasonography is an important tool in evaluating the seminal tract and, especially, in the diagnosis of prostate cysts. The centrally located cyst is not the only cause for hemospermia, but in all 10 patients in whom these cysts were unroofed, hemospermia disappeared within a mean of 16 months. To improve accuracy of the procedure and, more importantly, to avoid unwanted resection, TUUS is a valuable modality. In the diagnosis and treatment of hemospermia associated with prostate cyst, TRUS can be the optimal diagnostic tool, and during the unroofing procedure, TUUS can be of help.
