Abstract
- Huneke noted that needling or infiltration of scar tissue caused radiating pain, which has no apparent connection to the scar, and the subsistence, he referred to as Sekundenph nomen. A 72 years old female patient had a large scar on her lower abdomen. She had suffered intermittent pain on her abdomen and flank. The pain started around the urethra 6 months earlier, and was followed by dysuria and urinary frequency. When dry needling of the scar was performed with a 23 G hypodermal needle, pain developed on the lower abdomen and around the scar, but not around the urethra. One week later, the pain around the urethra had decreased from 7 to 3 on the visual analogue scale (VAS), but the dysuria and urinary frequency persisted. We blocked the lumbar sympathetic nerve, bilaterally, with bupivacaine under C-arm fluoroscopy. The dysuria and urinary frequency were almost eliminated.
