Experience with Penile Prosthesis: A Report of 16 Cases

Abstract

The penile prosthesis was implanted in 16 patients of erectile impotence at the Department of Urology, Chung-Ang University Hospital between November 1983 and June 1985. The results obtained were as follows. 1. The average patient age was 42.7 years, with a range of 28 to 63 years. The fifth decade(43.8%) was most followed by the sixth decade (25.0%) and fourth decade (18.8%). 2. The causes of erectile impotence were organic in 14 cases (87.5%) and psychogenic in 2 cases (12.5%). 3. The malleable penile prosthesis was inserted in 13 patients and inflatable penile prosthesis in 3. Of the malleable prosthesis implanted over 70% were 18-20cm long and the 9.5mm diameter was chosen in 12 cases (92.3%). Out of the inflatable prosthesis implanted, the cylinders were 16cm long with 2cm rear tip extenders in 2 cases, and the cylinder 18cm long with 1cm rear tip extender in the other one case. 4. The malleable prosthesis was implanted mostly (84.6%) under local anesthesia and mostly (84.6%) through perineal incision. Average operation time was 54 minutes. All the inflatable prosthesis were implanted under general anesthesia and through penoscrotal incision. Average operation time was 4.1 hours. 5. The average postoperative hospital stay was 8 days in malleable prosthesis and 9 days in inflatable type. There were 3 cases of early complications which occurred in the malleable prosthesis; prolonged pain for 8 weeks in 1 case, disruption of surgical wound in 1 and voiding difficulty in I. There was no late complications in the malleable and inflatable prosthesis. Thirteen cases (81.3 %) claimed their sexual satisfaction. Therefore, the results indicate that the implantation of the penile prosthesis, particulary malleabletype is to be a safe and effective method for the treatment of erectile impotence.
