RHINOPLASTY TO CLEFT LIP PATIENTS
Abstract
- The surgical treatment of the nasal deformities associated with cleft lip and plate patients was quite difficult, because of more composite deformed nasal complex. Usually, the nasal deformity is corrected at the time of primary cleft lip repair by means of nasal tip transfixation. But the results were never satisfactory esthetically. Surgical methods and timing for correction of nasal deformities of these patients were still controversy. Bardach and Sayler(1987) mentioned open thinoplasty with more active approach, not conservative, that could get a better results. According to severity of deformities of nasal complex and how can the patient feel that deformities internally, we can get a surgical method and get timing of rhinoplasty. It is essential of careful analysis of the anatomical structures, including the nasal complex and maxillary segments as well. We have treated with 7 patients complained of nasal deformities related to cleft lip and palate patients, aged 4 to 23 years old, by means of open rhinoplasty. We made a colummella and marginal incision and excised the excess lateral cartilage on affected side of alar nasi correct to straight that as possible, used the ear or septal cartilage, sometimes allogeneic cartilage graft. As a results, the patients were satisfied with quite symmetry of nasal complex and colummella lengthening after operation.
