Damage Control Laparotomy

Abstract

Currently ever-increasing numbers of trauma victims and the change in the spectrum of injury severity present new challenges to all trauma surgeons. Traditional surgical approach to all injuries includes sequence of steps just like an elective operation. However in severely injured patients with exsanguination the results were formidable because of accompanying lethal triad of metabolic acidosis, hypothermia and coagulopathy. In damage control laparotomy an abbreviated surgery is performed to minimize physiologic perturbations and after the patient is stabilized hemodynamically in the intensive care unit, a definitive management is carried out. Pathophysiology of exsanguination, three separate parts of damage control and postoperative problems are reviewed and discussed.
