Extramammary Paget's disease is a rare cutaneous adenocarcinoma, usually of epidermal origin and glandular differentiation and frequently associated with an underlying adnexal carcinoma and perhaps with underlying internal malignancy. We report a case of extramammary Paget's disease on the left scrotum and groin of a 73-year-old man who has a rectal carcinoma. He presented with a three months history of an ill defined, pruritic patch on the left scrotum and groin. On histopathological examiantion, we could observed many Paget cells confined to the epidermis and the hair follicle, but not invading the underlying dermis. Digital rectal examination revealed a 1.2X0.8cm-sessile villous tumor located in the anal canal and lower rectum. Histologic examination of the excised lesion revealed the presence of an adenocarcinoma with villous adenoma.
