Introducing and applying an intentional rounding program for nursing students in adult nursing practicum: Process and outcomes

Abstract

Purpose
Opportunities for nursing students to engage in face-to-face interactions with patients during clinical practicum have decreased. Because of this, changes in practicum content are necessary to ensure that nursing students can develop the competencies required in clinical settings, even under limited conditions. This study aimed to introduce a clinical practicum for nursing students that applied intentional rounding (IR).
Methods
A customized IR for students was integrated into the adult nursing practicum curriculum. Among students who participated, those who consented to join this study were included for outcome evaluation. Outcome variables included perception of patient rounding, nursing handoff competency, clarity of interprofessional communication, fall prevention activities, and clinical practicum-related stress. The study compared pre- and post-scores using paired t-tests. We analyzed IR records completed by the students.
Results
Significant improvements appeared after the application of IR in nursing handoff competency (t= -2.89, p=.005), clarity of interprofessional communication (t= -2.12, p=.038), and fall prevention behaviors (t= -6.33, p<.001). Furthermore, stress related to clinical practice significantly decreased (t=2.99, p=.004). Students completed the records thoroughly, although only a few students identified and reported patients’ needs or problems.
Conclusion
This study shows the importance of applying a customized IR. Even with short-term application (twice daily for one week), the program proved appropriate and effective. With minor adaptations reflecting specific ward and course objectives, this program can be expanded and successfully implemented in other nursing schools.
