Theoretical considerations of deep brain stimulation programming

Abstract

Deep brain stimulation (DBS) is an effective surgical option for medically intractable movement disorders. Adjustment of DBS parameters is the only modifiable factor that affects the clinical outcomes of DBS. It is very important to understand the basic functions of the basal ganglia and thalamus for successful DBS programming. The amplitude, frequency, and pulse width define the volume of tissue activated, the pattern of artificial action potentials, and the selectivity of activated neural elements, respectively. Based on this knowledge, it is possible to obtain the maximum clinical benefits and minimum side effects by appropriately adjusting the electrical stimulation to the target structure.
