Diagnostic Value of Technetium 99m-DTPA Radioaerosol Ventilation Scan in a Variety of Endobronchial Diseases

Abstract

To evaluate the diagnostic value of Tc99m-DTPA radioaerosol ventilation scan for prediction of the site and the character of endobronchial lesion and indirect prediction of ventilation-perfusion mismatch, we obtained Tc99m-DTPA radioaerosol ventilation scan and Tc99m- MAA perfusion lung scan in 8 patients of lung cancer, 3 patients of bronchiectasis, 1 patient of endobronchial tuberculosis and 1 patient of pneumonia. The results were as follows: 1) Sensitivity of ventilation scan according to anatomic lesion was 13 of 13 patients (100% ) and specificity was 11 of 13 patients(85%) and accuracy was 24 of 26 patients (92%). 2) Sensitivity of perfusion scan according to anatomic lesion was 13 of 13 patients (100%) and specificity was 9 of 13 patients(69%) and accuracy was 22 of 26 patients(85%). 3) In arterial blood gas analysis of matching group between ventilation scan and perfusion scan. PaO2 was 87.25 ± 8 .77 mmHg and PaCO2 was 35. 5±3. 79 mmHg. In mismatching group, PaO2 was 83 ± 12.99 mmHg and PaO2 was 36. 75±2. 5 mmHg. 4) In 2 of 8 patients with lung cancer. hot spot was noted on ventilation scan and perfusion defect in perfusion scan was noted in the corresponding region to hot spot in ventilation scan. We concluded that Tc99m radioaerosol ventilation scan may be valuable for detection of site and character of endobronchial lesion.
