Acute Massive Pulmonary Embolism Improved by rt-PA Thrombolytic Therapy -Two Cases of Report-

Abstract

Acute massive pulmonary embolism is a serious disease with high mortality rate if proper management is delayed. Thrombolytic therapy is indicated to coπect the hemodynamic derangement Urokinase and streptokinase are conventional drugs for this purpose, but their nonspecific systemic fibrinolytic activation can cause serious bleeding. Recombinant tissue type Plasminogen Activator (rt-PA) activates fibrin-bound plasminogen selectively, and is advocated as a local thrombolytic activator without systemic side effect. But there is little clinical experience on the usage of rt-PA in acute massive pulmonary embolism. We report two cases of acute massive pulmonary embolism improved obviously after rt-PA thrombolytic therapy together with review of literatures.
