The Introduction and Operation of HFACS (The Human Factors Analysis and Classification System)

Abstract

Around 70% of fatal accident causal factors are human factors. The human errors can be managed if the root causes were identified through the HFACS. If these human errors are not detected by flight crews, these could cause incidents or accidents. The HFACS has developed to examine human factors and improve accident investigation. The HFACS may detect and provide the causal factors so that the regulators and operators may improve latent conditions of the organization through effective strategies once the causal factors are identified. This paper is to introduce HFACS for commercial airlines, specially low cost air carriers. So the airlines improve safety and operate the SMS (Safety Management System) effectively using the analysis by HFACS.
