Abstract

Supracondylar fracture is the most frequent injury of the elbow and frequently accompany complications in children. In the treatment of a supracondylar fracture of the humerus in children, the aims should be a perfect anatomical reposition of the fragment attained by closed reduction. Il for any reason, it is found impossible to achieve this ideal. Four Supracondylar fractures which could not reduce satisfactory by closed method attempted under general anesthesia have been treated with open reduction and internal fixation with K-wire from a posterior approach from May 1972 to Dec. 1973. By operation findings, the difficulty in achieving a correct reduction is determined by: (1) the small size of the distal fragment, (2) the intensity of local edema, (3) anteroposterior obliquity, (4) tense hematoma, (5) many directions of displacement, (6) the special configuration of the fracture surfaces.
