A Case of Verrucous and psoriasiform Tinea Corporis by Trichophyton rubrum

A 34-year-old womar was admitted to the department of neurology, Seoul City Boramae Hospital for altered rnentality, food refusal and general weakness. She was a vagabond and had received anti-convulsant and anti-psychotic therapy over the past several years. She was referred to the department of dermatology for evaluation of skin lesions. There were light brownish, verrucous, hyperkeratotic plaques on the dorsum of both hands, wrists and soles, and pdoriasifom plaques on both elbows. Other skin lesions such as tina pedis, tinea crurir, and plaques on both elbows. Trichophyton ruhrum was isolated from the dorsum of the hand, elbow, ingui nal fold, sole, and fingernail. On histopathologic examination, papillomatosis and hyperkeratosis were observed from the hand lesion and psoriasiforrn acanthosis from the elhow lesion. She was anergir to a Multitest CMI examination. When richophytin was injectecl intradermally, wheal de veloped after 20 minutes but induration did not developed untill 72 hours. We report this case because of its rare clinical manifestation and the interesting restilts of the immunologic test.
