In congenital malalignment of the big toenail, the nail plate is deviated laterally with respeet to the longitudinal axis of ihe distal phalanx. In addition the nails exhibit transverse ridging, discoloration, thickened with gradual tapering of the distal portion. We present a case of a 70-year-old male who has had this nail problem for 7 years. The appearance of his nail was the same as congenital malalignment of the big toenail except that it was acquired.
