Long-Term Results of Penile Vein Ligation for Venogenic Impotence

Abstract
- Because penile vein ligation usually is associated with poor long-term results, a study was done of the efficacy of this procedure in patients having follow-up ranging from 24 to 39(mean 34) months. Eleven men with organic impotence documented by dynamic pharmaco-cavernosometry and cavernosography to have venous leakage underwent penile vein ligation. The patients' ages ranged from 27 to 47 years(mean 35) sustained potency without adjunctive therapy was present in 4 patients(36%). Of the 7 failures 2 had partial improvement in erections and 5 had no improvement, of whom 2 had gone on to penile prosthesis placement. Associated complications included penile shortening in 3 patients, penile deviation on erection in 2 and numbness at the base of penis in 1. We conclude that the long-term success of penile vein ligation is poor despite aggressive surgery. Therefore, a fundamental change in our treatment policy is necessary.
