Uterine Rupture During Labor

Abstract

This study is the clinical analysis of 43 cases of ruptured pregnant uterus at Chonnam University Hospital from jan. 1, 1980 to Dec. 31, 1996. The results were as follows: 1. The incidence of uterine rupture was 0.22%, 1 case per 450 pregnancies, and pure incidence(excluding referred patients from local clinics) was 0.1%, 1 per 1018 ones. 2. The highest distribution of age, gestational age and fetal weight were between 30 and 34 years old(44.19%), more than 39 weeks(72.09%) and 3000g(60%), respectively. 3. The most common cause of uterine rupture was previous cesarean section and its incidence was 77.27% 4. The frequent signs and symptoms of uterine rupture were loss of station, cesation of uterine contraction, inaudible fetal heart sounds, shock, lower abdominal pain and genital bleeding. 5. The interval between previous uterine injury and uterine rupture was within 24 months in 68% of cases. 6. 24 women among 34 cases of uterine rupture were referred to Chonnam University Hospital from local clinics. 7. There was no maternal death, but fetal death rate was 69.7% and pure one(excluding referred patients) was 31.6%. 8. We had performed debridement and simple repair in 18, total abdominal hysterectomy in 10 cases and subtotal hysterectomy in 1 case. 9. There were 5 cases of successful deliveries after simple repair of 21 ruptured uterus.
