Clinical Evaluation on 40 Cases of TLH ( Total Laparoscopic Hysterectomy ) and Comparison with Total Abdominal Hysterectomy

Abstract

A clinical evaluation was attempted to analyze 40 cases of TLH which was performed at Seoul Adventist Hospital from January 1997 to August 1997, and to compare them with total abdominal hysterectomy of 75 cases at the same hospital. The results of this study can be summarized as follows. 1. The mean age of the patients for TLH was 45.9 years. 2. The most common indication for TLH was myoma (55%), and other indication were adenomyosis (32.5%), myoma with adenomyosis (7.5%) and CIS (5%). 3. The mean operative time for TLH was 126.6+/-33.7 minute (80~245 min). 4. The mean estimated blood loss for TLH was 210.2+/-101.2 ml (30~800 ml). 5. The mean uterine weight for TLH was 222.2+/-98.6 gm (100~849 gm). 6. The mean hospital stay of TLH was 6.1+/-1.0 days. 7. The complications were intraoperative bleeding (2 cases), trocar site hemorrhage (1 case), headache (1 case) and disuria (1 case). In our conclusion, TLH technique shows minimal postoperative hemorrhage, postoperative discomfort, low incidence of complications and short duration of hospitalization. TLH will be a standard operation for hysterectomy in the near future, but the procedures demand sophisticated instrumentation and a dedicated endoscopyteam to ensure safe and efficient performance.
